Needed Items to Screen

1. Screening Tool

Make a telephone script that addresses the specific criteria you are looking for in the person for your particular position (ie. aide, receptionist, etc.)

2. Application for Employment

Make sure to ask for NON-personal references and call them.

3. Job Description

It's good to have a general description that emphasizes your expectations in regards to reliability, flexibility, etceterra as well as a specific one that describes their exact roles and day-to-day tasks.  It's good to not give second chances and be up front with the potentially worst part of their job (and the best).  Attempt to discover what part of your expectations they may have the easiest and the most difficult time with.
